The Opportunity: We are looking for a Rehabilitation Services Coordinator to join a community-focused team supporting individuals with disabilities and special needs. This opportunity involves coordinating services, promoting community participation and inclusion, and supporting individuals with cognitive, physical, and/or mental health-related needs.
Schedule: Various hours for business needs
Hours: 35 hours weekly
Wage: $28.43 per hour
Responsibilities: Coordinate services and supports for individuals with disabilities and special needs.
Assess individual needs and identify effective compensatory strategies to maximize community participation and inclusion.
Supervise and train employees, including organizing, prioritizing, and scheduling work assignments.
Apply behaviour management principles to support individuals effectively.
Use problem-solving and conflict-resolution skills to address challenges.
Work collaboratively with individuals, families, staff, and community partners.
Promote positive human relations and respectful, person-centred service delivery.
Ensure compliance with applicable Health & Safety legislation.
Maintain effective written and verbal communication.




Use computer hardware and software to support service coordination and documentation.
Qualifications: Bachelor’s degree in Human Services or a related program is preferred.
2–5 years of experience in the Social Services or Health Sector, preferably coordinating services for individuals with disabilities.
Knowledge of cognitive impairments and mental health diagnoses.
Community-based experience working with individuals with cognitive impairments, physical disabilities, or special needs.
Strong written and verbal communication skills.
Highly organized with excellent time-management skills.
Demonstrated ability to supervise and train employees.
Solid problem-solving and conflict-resolution skills.
Knowledge of human relations.
Knowledge of behaviour management principles.
Knowledge of Health & Safety legislation.
General understanding of computer hardware and software.
Strong ability to assess individual needs and determine effective compensatory strategies that promote maximum community participation and inclusion.
