In this front-line role, the Registered Practical Nurse (RPN) directly contributes to achieving the mission of the hospice. You will conduct comprehensive assessments and create personalized care plans for patients, working alongside physicians and support staff. Your expertise will be key in managing complex clinical situations and facilitating a positive environment for patients and their families.

Key Responsibilities:
• Conduct ongoing patient assessments using the CHPCA framework
• Develop and evaluate personalized care plans




• Provide hands-on care including medication administration
• Identify and manage palliative care emergencies
• Educate patients about their condition and care options

Requirements:
• Registration in positive standing as an RPN
• Three years' experience in palliative care
• Knowledge of pain and symptom management
• Excellent communication and critical thinking skills
• Flexibility to work varied hours
