Northern Health is committed to fostering diversity and seeks an Indigenous Care Coordinator to work within a multi-disciplinary team. You will create and monitor personalized care plans, ensuring cultural safety and community support throughout the patient’s healthcare journey. Your role will also involve educating the care team on Indigenous wellness approaches and coordinating with First Nations and Métis communities.

Key Responsibilities:
• Design and implement patient care plans for Indigenous individuals
• Collaborate with multi-disciplinary healthcare teams




• Provide education on Indigenous cultural safety and wellness
• Coordinate services with First Nations and Métis communities
• Support patients from intake to post-discharge

Requirements:
• Indigenous ancestry is strongly preferred
• Experience in healthcare coordination is an asset
• Knowledge of Indigenous wellness and trauma-informed care
• Strong communication and collaboration skills
• Ability to work Monday to Friday, 08:00 to 16:00
