LOFT Community Services is seeking a Case Manager for the M-DOT Hotel Team in Toronto to provide direct service to individuals living with mental health, health and addictions challenges. The role includes in office and community assessments and collaboration with a multidisciplinary team to support housing and health needs.
You will develop individualized support plans, connect clients with services (housing, ID, OW/ODSP), coordinate care, and participate in crisis intervention.
