Job Description ACCOUNTABILITIES: Case Work/Facility Maintain an active caseload.



Responsible for the day-to-day shift duties and reporting requirements. Complete intake and assessments, creating service plans, providing case management services and clinical interventions for an assigned caseload. Maintaining ongoing responsibility for primary client housing-stability planning and housing-related case work. Record activities and incidents occurring during the shift in a factual and concise manner using Logbook and/or Incident Occurrence Reports. Contact emergency service personnel (ambulance or police) when necessary. Maintain confidentiality with regard to clients/staff and Centre's records. Develop a network and make referrals to existing services and resources in the community including those of other Salvation Army departments. Maintaining personal HIFIS
