Vermilion & District Housing Foundation is seeking a dedicated full time Licensed Practical Nurse (LPN) to join our healthcare team, providing compassionate and comprehensive patient care across various settings. The ideal candidate will have experience in diverse clinical environments, including acute care, long-term care, assisted living, and geriatrics. The LPN will collaborate with multidisciplinary teams to deliver high-quality care tailored to patient needs while maintaining accurate medical documentation.

Duties

- Conduct thorough patient assessments including vital signs, physical examinations, and physiological evaluations.
- Administer medications, injections and perform catheterizations following sterile techniques and aseptic procedures.
- Document all patient interactions accurately.
- Provide patient education on health coaching topics including medication management and chronic disease care like dementia or Alzheimer's.
- Ensure infection control protocols are strictly followed to prevent healthcare-associated infections.
- Collaborate with Clinical Director, Homecare, physicians and other healthcare professionals.
- Support patients with disabilities or developmental challenges by delivering specialized care tailored to their needs.

Skills





- Proficiency in medical terminology and anatomy knowledge for accurate patient assessment and documentation.
- Strong understanding of infection control practices and aseptic techniques.
- Ability to perform physical examinations and diagnostic evaluations accurately.
- Knowledge of specialized areas such as geriatrics, pediatrics, behavioral health, hospice care, home & community care.
- Excellent patient care skills with the ability to work effectively with diverse populations including children, elderly patients with dementia or Alzheimer's disease, individuals with disabilities or developmental disorders.
- Critical thinking skills for triage situations and acute pain management; capable of working under pressure in fast-paced environments.
