Position: Intensive Case Manager, Unionized Position
Reports to: Prevention Outreach Manager
Location: Halifax & Dartmouth, Nova Scotia
Working Hours: Monday to Friday, 9:00am-5:00pm (80hrs bi-weekly)

Coverdale Justice Society is a community-based organization dedicated to improving access to justice, promoting alternatives to traditional justice systems, and enhancing public safety and well being, with a focus on primarily serving communities of criminalized women and gender-diverse individuals, including Indigenous and African Nova Scotian women, trans and queer people, and those affected by gender-based violence and poverty.

Position Summary
Reporting to the Prevention Outreach Manager, the Intensive Case Manager helps women and gender-diverse individuals navigate the justice system, homelessness, and related systems within the Halifax Regional Municipality by connecting them, quickly and effectively, to the right community resources, services, and supports.
This role is centred on system navigation, resource brokering, and advocacy.
The Intensive Case
Manager draws on extensive, current knowledge of the community-based resources, programs, and services available across Halifax Regional Municipality, and on a strong network of relationships with the organizations and community partners that provide them, to link clients to housing, legal, health, income, and social supports aligned with Coverdale Justice Society's mission, vision, and values.
The Case
Manager maintains a caseload and develops individualized, strength-based navigation plans while supporting clients in securing and maintaining safe and stable housing.
Core Duties and Responsibilities
- Manage a caseload of 10–12 individuals, providing consistent weekly navigation support.
- Serve as clients' primary point of connection to the network of community, government, and social services available across the Halifax Regional Municipality.
- Maintain current, working knowledge of community resources, eligibility criteria, and referral pathways across housing, legal, health, mental health, addictions, income assistance, employment, and cultural/community supports.
- Build, maintain, and continually expand a personal and organizational network ofrelationships with community organizations, government agencies, landlords,healthcare providers,
and other service providers.
- Develop individualized, strength-based navigation and case plans utilizing HousingFirst and harm reduction principles.
- Provide referrals and active follow-up, not just a hand-off, to ensure clients successfully connect with and access the resources they're referred to.
- Liaise and collaborate with Coverdale Supportive Housing staff, including Frontline Staff,Case Managers, House Managers, and Reintegration Coordinators, to support the intakeand transition of community clients into Coverdale housing programs.
- ● Collaborate with housing staff to support eviction prevention, housing stability, andappropriate interventions for current residents experiencing challenges that may impacttheir housing.
- Support coordinated planning and communication when residents are transitioning fromCoverdale housing following eviction, discharge, or completion of their housing placement, including transitions to independent housing or other supportive housing options.
- Work directly with residents of Shannon’s Place to provide case management, facilitate programming, and support individualized housing plans that assist residents in progressing through the program toward successful, long-term independent housing.
- Assist clients in navigating housing, legal, and income systems, including completing applications (housing, ID, income assistance, legal aid, and similar) and attending appointments and lease signings.
- Provide intake, assessment, referrals, crisis intervention, eviction prevention, outreach, and mediation.
- Advocate for clients' rights and needs at both the individual and systemic level, including with service providers and institutions.
- Support individuals transitioning out of homelessness and justice involvement to locate, secure, and maintain appropriate housing.
- Provide in-home and community-based support to promote stability.
- Support clients with obtaining essential items required when transitioning into new housing, including financial management in accordance with organizational Finance policies.
- Maintain accurate and up-to-date case files, resource and referral records, andrequired program documentation.
- Complete documentation and reporting within HIFIS in accordance withorganizational and funder requirements.
- Represent Coverdale at community tables, partnership meetings, case conferences, andcommittees to strengthen the organization's community network and stay current onavailable resources.
- Work collaboratively with Coverdale team members and external communitypartners to provide coordinated, client-centred support.
- Maintain confidentiality, privacy, professional boundaries, and adherence to CoverdaleJustice Society policies and procedures.
- Contribute to program development and the ongoing improvement of client-centred programs and services, including identifying gaps in the community resource network.
- Perform other related duties as required.
Education, Knowledge, Skills, and Experience
- A post-secondary degree or diploma in a relevant social services field, such as Social Work, Counselling, Psychology, Criminology, Social Services, or a related discipline. An equivalent combination of education and experience, including professional and volunteer work may also be considered.
- Experience in case management, systems navigation, frontline service delivery, program facilitation, and/or conflict resolution while supporting high-acuity individuals experiencing complex mental health, substance use, and homelessness related challenges.
- Demonstrates extensive knowledge of the community-based resources, programs, social services, and non-profit sector within the Halifax Regional Municipality, with a proven ability to connect clients to the right supports in a timely manner.
- Strong networking, relationship-building, and community engagement skills, with established connections across community agencies, service providers, and government partners is considered a strong asset.
- Working knowledge of and experience applying anti-oppressive, human-centred, and trauma-informed frameworks and practices, including when working with people with diverse and overlapping needs.
- Strong organizational and time-management skills, with the ability to manage competing priorities and work both independently and collaboratively.
- Proficiency in data collection, case documentation, and electronic reporting systems, including HIFIS or similar data management platforms.
- Proficiency and comfort using Microsoft OfXice/OfXice 365, Google Workspace/Google Drive, databases, virtual communication platforms, and other workplace technologies.
Employment and Working Conditions
- Employment: Full-time, permanent, unionized position
- Working Hours: Monday to Friday, 9:00am-5:00pm (80hrsbi-weekly)
- Wage: $29.09/hr ($60,507 annually)
- Location: work is based throughout Halifax and Dartmouth. Includingcommunity-based locations providing support in clients’ homes and throughout thecommunity.
- Valid Nova Scotia driver’s licence required.
- Access to a reliable vehicle with appropriate insurance is required, as transportation of program participants is needed.
This position is part of a unionized work environment and is subject to the termsand conditions outlined in the applicable collective agreement.Benefits
