Position Summary

Full-time: 38.0 hours weekly The Licensed Practical Nurse (LPN) utilizes the nursing process in collaboration with the multidisciplinary team in the delivery of consistent quality care to patients with a variety of health concerns. The LPN supports the primary care physician in the assessment, planning, implementation and evaluation of care for the purpose of promoting, maintaining or restoring health, and preventing illness or injury.

Key Responsibilities

· Assist physicians and team members with patient care, including assessing, managing and supporting patients

· Obtain, update and maintain patient histories and medical information on the electronic medical record (EMR) . Our clinic uses AVA.

· Facilitate clinic workflow by showing patients to exam rooms, taking vital statistics and assisting physicians with procedures, as needed

· Provide patients with information about health related issues/concerns, medical care, routine screening and life styles options

· Assist with patient follow-up care by scheduling appropriate tests, appointments and processing referral requests

· Assist in evaluating the effectiveness of patient care through individual patient evaluation

· Documents assessment and care plan in the patient electronic record

· Communicate essential information to referring physician in a timely manner to support seamless care.

· Collaborate with the program manager and team and to identify and develop unique approaches to address patient needs and maximize patient care outcomes

· Mentors and orientates new staff to program/service area

· Participates in meetings, ongoing program development, evaluation and quality improvement opportunities





· Assist with implementation of new protocols and act as a resource for other staff

· Performs additional duties as required

Qualifications

Education/Certification:

· Completion of an accredited Licensed Practical Nurse program

· Current registration with CLPNA

· Current CPR certification

Experience:

· Two (2) to five (5) years’ clinical experience preferred

· Experience working in an acute, outpatient or community practice setting specifically with chronic disease or obstetrical focus

· Proficient with computers and technology; experience with Netcare, EMR preferred

Working Conditions:

· Normal clinic environment with a moderate degree of exposure to aspects of the work environment.

- Can be quick paced and busy with eight doctors.

Job Types: Permanent, Full-time Pay: From $26.00 per hour
