Nurse Practitioner – Primary Care (Part Time – Permanent)

The Role and Opportunity The Primary Health Care Nurse Practitioner plays a key role in providing comprehensive primary health care services to individuals accessing care through the CMHA Nurse Practitioner Clinic. Services are delivered in the clinic setting and, when required, in the community.

The Nurse

Practitioner provides holistic, client-centred care through health assessment, diagnosis, treatment, and follow-up across the lifespan. The role also includes health promotion, chronic disease management, collaboration with inter-professional team members, and participation in program development and quality improvement activities.

This 0.5 FTE position offers flexibility in scheduling, making it an excellent opportunity for candidates seeking to balance other professional, academic, or personal commitments.

Salary: $59.24 per hour – $68.99 per hour

Hours of Work: 20 per week

Location: 700 Lawrence Ave West, North York

Responsibilities

- Conduct comprehensive health assessments, develop care plans, and provide ongoing primary health care to clients across the lifespan.
- Diagnose, treat, and monitor acute and chronic health conditions within the Nurse Practitioner scope of practice.
- Prescribe medications, order and interpret diagnostic tests, and implement evidence-based treatment plans.
- Provide health promotion, disease prevention, chronic disease management, and wellness education to clients and families.
- Coordinate care and facilitate referrals with physicians, specialists, hospitals, community agencies, and other healthcare providers.
- Support clients in accessing health, social, and community resources that promote overall well-being.




- Provide education and support to families, caregivers, and substitute decision-makers regarding treatment, health conditions, and available resources.
- Collaborate with inter-professional team members to deliver integrated, client-centred care and improve health outcomes.
- Contribute to program development, clinical education, mentorship, quality improvement, and evidence-based practice initiatives.
- Maintain accurate clinical documentation and ensure compliance with professional, organizational, and regulatory standards.

Experience & Education Required

Education

BScN Degree and Registration with the College of Nurses of Ontario and NPAO

Related Experience

- Experience in providing counselling to clients around issues related to medications and their effects
- Minimum 5 years of related clinical practice in community mental health and primary health care setting.
- Experience providing a consultative/educative service from a nursing perspective to other service providers and other community agency staff

- Demonstrated knowledge or the CNO Standards of Practice and the NP’s expanded scope of practice, based on legislative authority
- Demonstrated critical thinking, self-direction, decision making and problem-solving skills
- Proficiency with computers, MS word, Internet searches, data entry and client information systems
- A valid Ontario driver’s license and access to a vehicle is required for this position
