Nurse Practitioner-Led Clinics are a primary health care delivery model in which Nurse Practitioners are the lead providers of primary health care. Our clinic will provide service to those clients who do not have a primary health care provider or choose to use the clinic for all of his or her primary care. Clients are offered a variety of services through an interprofessional team of Nurse Practitioners, Registered Nurses, Collaborating Physicians, Physiotherapists, Dietitians, Social Workers and Health Promoters.

Our clinic is focused on improving quality of care by integrating primary care with mental health services, health promotion, case management, disease prevention and chronic disease management.

Position: 1 FTE The Registered Physiotherapist works in collaboration with nurse practitioners, members of interprofessional team members to provide client-centred, quality primary health care services. Youwill work closely with our interprofessional team to support the attachment of patients to our clinic. You will have opportunity to support the development of services as part of our expansion The Registered Physiotherapist will practice within his or her scope of practice as outlined in the Standards of Practice by the College of Physiotherapist of Ontario.

Major Job Responsibilities:

· Provide community-based physiotherapy services within full scope of practices outlined by the College of Physiotherapists of Ontario, including but not limited to assessment, diagnosis and education intervention

· Diagnoses and assist diverse clients with acute and chronic health conditions and rehabilitation needs, from the lens of chronic disease prevention and management

· Provide physiotherapy services independently and within a team, including rehabilitation assistants

· Ensuring practice is consistent with professional standards and the ethical framework as outlined by the College of Physiotherapists of Ontario and organizational policy, practice and values





· Demonstrating commitment to professional lifelong learning by participating in continuing education activities to meet the needs of the patient population served and the standards of the profession

· Performing reflective practice, analyzing and evaluating physiotherapy knowledge and modifying practice accordingly to improve the quality of patient care

· Performing cross-functional and other responsibilities, as assigned and/or requested

· Maintain accurate and timely clinical documentation adhering to agency standards

· Participate actively in program development and evaluation

Skills & Qualifications:

- Master of Science in Physiotherapy or equivalent
- Member in good standing with the College of Physiotherapist of Ontario.
- 3-5 Years of clinical experience in community setting
- Strong interpersonal, organizational and problem-solving skills
- Shown capacity to make independent judgements and work without supervision
- Demonstrated ability to work in a team, work independently and work within a community practice setting.
- Proficiency in the use of Windows, Microsoft Office 365 and PowerPoint.
- Excellent written and oral communication.
- Proficiency in English, a second language reflecting local community need (such as Cantonese and/or Mandarin) is an asset
- Demonstrated ability to work in a multi-cultural setting.
- Demonstrated commitment to continuous learning
- Participate in annual performance review and learning plan.
- Able to work effectively with all members of the inter-professional team
- Able to prioritize, manage time effectively and be flexible in a very active work environment
- Experience with EMR, Telus PSS would be an asset.
- A satisfactory result of Vulnerable Sector Police Record Check is a requirement of this position.

Work Conditions:

1. 1 FTE, 37.5 hours per week.
2. Travel within the community may be required.
3. Able to work flexible work hours, including some evenings and weekends
4. Work in all NPLC locations when needed.

Compensation: Salary$83,987 Annually
