This is to fill a position with the Elgin Ontario Health Team (Elgin OHT).

The Elgin Ontario Health Team (Elgin OHT) is made up of organizations, clinicians, and community members working together to improve the health and wellbeing of the people of Elgin. Together, we are reimagining how to provide care in a more integrated, coordinated way that better serves and supports our community.

We are seeking a Primary Care and Community Engagement Coordinator to support initiatives that strengthen primary care access, clinician engagement and community awareness. Reporting to the Elgin OHT Executive Director, this temporary contract position will provide coordination, communications, engagement and project support for established OHT and Elgin Primary Care Network priorities through March 31, 2027.

WHAT YOU WILL BE DOING:

- Support delivery of a public education and promotional campaign to help Elgin residents understand how to find a family doctor or nurse practitioner and the value of an ongoing relationship with a primary care provider.
- Develop, maintain and distribute print and digital communications materials using approved messaging, including presentations, website content, social media content and educational resources.
- Coordinate community and provider engagement activities, including logistics, communications, materials, and follow-up.
- Gather, track and summarize engagement feedback and activity metrics for reporting purposes.
- Coordinate provider-focused events, including the Elgin Primary Care New Provider Orientation, Continuing Medical Education events and other OHT or Primary Care Network activities.
- Conduct research and support documentation,



reporting and project activities related to OHT and Primary Care Network priorities.
- Support other projects and initiatives as assigned.

WHAT YOU MUST HAVE:

- College diploma in Communications, Public Relations, Health Promotion, Community Development, Health Administration, Business Administration, Social Services, or a related field. Equivalent combination of education and directly-related experience may be considered.
- A minimum of one (1) year of experience in communications, community engagement, project or event coordination, health promotion, knowledge translation, or a related field.
- Demonstrated creativity and innovative, solutions-focused approach.
- Demonstrated ability to build collaborative relationships and communicate effectively with healthcare providers, community partners, administrators and members of the public.
- Experience working with physicians and interdisciplinary teams
- Ability to interpret policies, conduct research and develop documentation based on the research data obtained.
- Exceptional organizational skills and ability to manage multiple priorities and deadlines.
- Advanced knowledge of computer applications (e.g. Microsoft Office, Zoom) and willingness to learn current applications
- Ability to regularly attend work in accordance with the Attendance Support Program
- Acceptable performance record that is aligned with OHT values and Code of Conduct

WHAT WE PREFER YOU HAVE:

- Knowledge of the Elgin community and local health and social service landscape is an asset.

HOURS AND WORKING CONDITIONS:

- This is a temporary full-time contract position ending March 31, 2027, subject to available funding.
- Hybrid role with regular in-person engagement across Elgin
- Flexible scheduling; must have access to personal transportation
