Duties

- Assess patient conditions accurately using medical terminology and clinical judgment to develop individualized care plans.
- Administer medications and treatments following established protocols, ensuring patient safety and comfort.
- Collaborate with interdisciplinary teams using electronic health record systems such as Epic or Cerner to document patient progress and coordinate care.
- Perform phlebotomy procedures and laboratory tests to support diagnostic processes.
- Manage pain effectively through appropriate interventions, including medication management and alternative therapies.
- Provide specialized care for pediatric, senior, dementia, or assisted living patients, tailoring approaches to meet unique needs.




- Conduct case management activities to ensure seamless transitions between care settings like hospitals, nursing homes, or home care.
- Assist with medical office tasks such as patient intake, charting, and medical terminology documentation to streamline operations.

Requirements

- Valid nursing license with current registration in Ontario
- Proven hospital or Long term care experience with a focus on acute care environments.
- Strong background in patient care across various settings including pediatrics, senior care, or assisted living facilities.
- Experience working with electronic health records (EHR) systems such as Epic or Cerner is highly preferred.
- Knowledge of medication administration protocols and medical terminology is essential.
- Demonstrated ability to perform laboratory procedures like phlebotomy and handle laboratory samples accurately.
- Excellent communication skills for effective collaboration with healthcare teams and patients’ families.
- Prior experience in pain management, dementia care, or home health services is advantageous.

Pay: $32.00-$50.00 per hour

Expected hours: 8.0 – 24.0 per week
