We are seeking a dedicated and compassionate Registered Practical Nurse (RPN) to join our dynamic healthcare team. Duties

- Assess patient needs and develop individualized care plans in collaboration with healthcare teams
- Administer medications and treatments accurately following established protocols
- Monitor patient vital signs, symptoms, and responses to treatment, documenting all observations thoroughly
- Assist with activities of daily living such as bathing, dressing, mobility support, and nutrition management
- Conduct specialized procedures including phlebotomy and laboratory specimen collection
- Provide dementia care, hospice support, and pain management to enhance patient comfort and dignity
- Utilize electronic health records systems like Epic and Cerner for documentation and communication




- Support case management efforts by coordinating services and liaising with families and healthcare providers
- Ensure compliance with medical terminology standards and maintain a safe environment for patients and staff

Skills

- Proven experience working in nursing homes, hospitals, or acute care settings with a focus on patient-centered care
- Strong knowledge of medical terminology, medication administration protocols, and laboratory procedures
- Skilled in providing dementia care, senior care, hospice support, and pediatric nursing when required
- Experience working with electronic health records such as Epic or Cerner is highly desirable
- Excellent communication skills for effective collaboration with multidisciplinary teams and families
- Ability to handle emergency situations calmly while delivering compassionate care
- Familiarity with assisted living environments, home care services, pain management techniques, and case management practices
- Knowledge of medical office procedures and experience in assisted living or hospice environments is a plus
