We are seeking a dynamic and compassionate Resident Care Coordinator to join our dedicated healthcare team. Duties

- Coordinate and oversee daily resident care activities to ensure high-quality service delivery aligned with individual care plans
- Collaborate with nursing staff, physicians, and other healthcare professionals to develop and update personalized care strategies
- Supervise and support caregiving teams, fostering a positive work environment that encourages teamwork and professional growth
- Manage case documentation, including assessments, progress notes, and incident reports using electronic health record systems such as Epic or Cerner
- Assist with medication administration, pain management protocols, and medical procedures like phlebotomy when necessary
- Monitor resident safety protocols, including dementia care practices and acute care procedures




- Communicate effectively with families regarding resident status updates and care plan modifications

Requirements

- Proven experience in hospital or senior care settings with a strong understanding of patient care standards
- Demonstrated team management and supervising experience within healthcare environments
- Knowledge of medical terminology, medication administration, and nursing practices
- Experience working with diverse populations including seniors, children, or individuals with dementia or special needs
- Familiarity with electronic health records systems such as Epic or Cerner is highly preferred
- Strong organizational skills with the ability to handle case management responsibilities efficiently
- Excellent communication skills to liaise effectively with residents’ families and multidisciplinary teams
