Your role will be to coordinate essential services that ensure independent living for the elderly population. As the Senior Care Coordinator, you will assist seniors aged 55+ by managing service delivery that includes friendly visiting and home support. This position requires tracking contracts for lawn care, snow removal, and more, while documenting all client interactions in our health tracking systems.

Your contributions will help facilitate vital transportation services for medical visits, reflecting the principles of the Métis Nation of Ontario. Key Responsibilities:

- Implement Aging at Home services with a focus on client needs
- Perform service assessments and coordinate external agency efforts




- Monitor contracted services such as lawn care and snow management
- Assist in accessing transportation for specialized medical needs
- Keep detailed records in the Online Health and Access Tracking System Requirements:
- At least 1 year of relevant client services experience
- Experience with the elderly population (55+)
- Skilled in Microsoft Office Suite applications
- History of non-profit volunteer work is advantageous
- Social services degree or diploma is preferred Facilitate effective aging services through exceptional coordination and unwavering commitment to the senior community.
