WoodGreen Community Services is seeking a part time Client Support Worker for the Cummer Housing Program in Toronto. The role focuses on assisting seniors experiencing homelessness with goals, daily living, and access to health and social services, in a supportive onsite setting.

You will build relationships, support care planning, manage intake, and help navigate housing resources, while promoting independence and community integration.
