Your responsibilities will include performing psychosocial assessments and short-term counseling for patients and families facing complex health challenges. You'll coordinate care plans in collaboration with healthcare professionals and address any service delivery gaps swiftly. Key Responsibilities:

- Complete comprehensive psychosocial assessments
- Provide short-term support and counseling for grief and end-of-life issues




- Develop patient care plans alongside healthcare teams
- Cover shifts for other Social Workers as needed
- Liaise with community health professionals for resource connections Requirements:
- Master's degree in Social Work
- Registration with Ontario College of Social Workers
- Recent hospital social work experience
- Strong communication and problem-solving skills
- Experience with electronic documentation, preferably Meditech Support patients through psychosocial challenges by joining the dedicated team at Oak Valley Health.
