Lead change as a Follow Up Support Worker at YMCA, focusing on empowering clients through the Housing First model in Edmonton. Provide hybrid case management and community connection. In this full time role, you will enhance housing stability for individuals who have transitioned into homes via the Housing First program.

With your trauma-informed approach, you will develop personalized action plans and conduct home visits ensuring clients succeed. Your work will make communities resilient and supportive. Key Responsibilities:

- Deliver trauma-informed follow-up support and advice
- Create and manage Individual Action Plans for clients
- Conduct wellness checks to ensure housing stability
- Advocate for access to community resources
- Support clients in using public transit for appointments Requirements:
- Degree or diploma in Social Work or related area
- At least 2 years' experience in social work contexts
- Valid driving license and access to a vehicle
- Exceptional organizational and communication skills
