We are offering long term contracts as well as Ad-Hoc shift work as well as competitive rates of pay and mileage.ResponsibilitiesCoordinating care for individuals with long-term conditions or complex health needsAdministering medication and supporting self-administrationDressing and managing woundsProviding Catheter Care in Clinics and within the individual's homeProviding Wound Care for housebound individuals after surgerySupporting issues around continenceCannulation / IV Therapy administrationCaring at end of lifeExperience6 months experience of working at relevant level within the community in the UKHas a full UK driving license and access to an insured car dailyAble to work within a diverse community of patients and health care professionals.Effective communication skills written,



verbal and non-verbal with the ability to adapt communication for individuals with communication difficultiesAble to make clinically reasoned judgements when working with a variety of conditions.Able to work autonomously, and ability to know when to seek appropriate advice / guidance from senior staff.Effective time management and caseload management.Positive problem-solving skills.Working knowledge of clinical outcomes measures, clinical audit.Experience with EMIS, undertaking assessments, providing holistic care to elderly patients on DN caseloadManagement of long-term conditions and palliative careAware of safeguarding adultsSearch for health needs and working with the core and wider MDT to meet patient needs
