Position Description:The Palliative Care Clinical Coordinator is a member of Kenora Chiefs Advisory's Primary Care Team. This position is intended to provide clinical coordination for patients and families undergoing Chronic disease and end of life illness.Position Title:Palliative Care Clinical Coordinator (new)Department:Primary CareStarting Salary:$84,365Term:Full timeLocation:240 Veterans Drive, 3rd Floor, Kenora, OntarioReports to:Director of HealthDuties and Responsibilities:The Coordinator will lead early identification of community members who would benefit from a palliative approach, support comprehensive care planning, coordinate the multidisciplinary care team, and strengthen transitions between hospital and community care.Work alongside Elders, traditional knowledge keepers, nurses, physicians, nurse practitioners, and regional partners to ensure care is wholistic, culturally secure, and aligned with First nation Community values.Meet with allied Health Professionals within the All-Nations Health partners to coordinate care and case management.Meet with Patients and families to coordinate care for Chronic disease management and end of life care/ comfort care.Provide primary navigation support to patients, families, and caregivers, and who will improve access to existing clinical and community services.Provide education to patients and family members on available resources and careEstablish relationships with Health Directors of First Nations communitiesReduce fragmentation and improve continuity of care by establishing reliable coordination and navigation,



mobilizing existing services more effectively, strengthening caregiver education and supports, and advancing a sustainability plan for comfort care across KCA communitiesPrepare summaries and reports after each session, from process content and evaluation as required by the Medical DirectorParticipate in KCA Primary Care Health Team activities as requiredContribute to the evaluation of service with respect to its effectiveness in meeting the community needs and achieving our mandateAdditional duties as requiredExperience working with an Electronic Medical RecordQualifications:Ability to work independently and in a team settingExcellent skills in the following areas: computer, organization, networking, interpersonal, presentation and communication (written and oral), Time ManagementExcellent communication skills.A full understanding of Anishinaabe culture, the ability to speak Ojibway is an assetUniversity degree or college diploma from a recognized post-secondary institution as Registered Nurse in good standing with the College of Nurses of Ontario.Additional Requirements:Valid driver's license and vehicle; Must be willingto travel when required; Vulnerable Sector Criminal Records Search.Contact Information:Human Resource DepartmentKenora Chiefs AdvisoryP.O.
