This is a shared role with:2 days per week at Cerwydden Care3 days per week at Oyster Harbour Care CentreIf you're passionate about person-centred care and thrive in a collaborative, mission-driven environment, we'd love to meet you.About the RoleAs the Social Worker, you will play a key role in supporting residents' psychosocial well-being while partnering closely with families, clinical teams, and external agencies. This role is ideal for someone who values relationship-building, advocacy, and continuity of care across multiple settings.Key ResponsibilitiesProvide psychosocial assessments, counselling, and emotional support to residents and familiesSupport residents during transitions, health changes, and end-of-life careAssist with care conferences, care planning, and interdisciplinary collaborationProvide guidance on community resources, financial supports, and advance care planningSupport admission, discharge, and adjustment processesAct as an advocate for residents' rights, dignity,
