This is a virtual position for the most part with room for face to face capacities when necessary or desired throughout New Jersey.Position Overview:We are seeking a compassionate and detail-orientedMedical Case Manager / Public Benefits Navigatorto join our team in Current Jersey. This role is ideal for someone with strong experience inpublic benefits enrollment, insurance navigation, and medical case management, particularly in complex and high-need situations.



The right candidate will be comfortable working directly with patients, navigatingcounty assistance offices, and helping individuals access critical services includingMedicaid, SNAP (food stamps), and other local/state programs .Key Responsibilities:Conduct in-depth assessments of patients eligibility for public benefits and medical resourcesAssist patients with enrollment in Medicaid, SNAP, and other public assistance programsTroubleshoot and resolve complex enrollment challengesCommunicate with county assistance offices, insurance providers, and healthcare systems on behalf of patientsCoordinate services and maintain up-to-date documentation and case notes in EHR systemAdvocate for patients facing barriers to care or benefit accessConnect to outside providers such as behavioral health specialists and drug and alcohol support programs.
