Enhance lives as a Registered Practical Nurse with our Mobile Response Team in Brockville, Ontario. Deliver compassionate care for older adults with responsive behaviours related to dementia and mental health challenges. This full time position at Providence Care involves direct collaboration with RNs and PSWs to provide critical interventions in long-term care settings.

You will assist in transitions and develop tailored care plans, leveraging your expertise in responsive behaviour management. Aiming for person-centred care, you’ll work with a dedicated team to improve the quality of life for clients. Key Responsibilities:
Provide direct care in collaboration with the MRT RN and PSWs
Develop assignments and support MRT PSWs
Collaborate with seniors/geriatric mental health teams




Utilize evidence-based assessment tools for client evaluation
Implement Team-oriented Care Plans for responsive behaviours Requirements:
Current RPN registration with the College of Nurses of Ontario
RPN Diploma with medication certificate
Minimum one year related experience with older adults
Experience in supportive management of responsive behaviours
