ongoing patient relationships over the phone and in-person.Collaborate with prescribers and other healthcare providers to ensure coordinated care.Document consultations and follow-ups in compliance with standards of practice.Stay current with diabetes guidelines and best practices to deliver evidence-based care.Support all responsibilities related to pharmacy operations including but not limited to medication/supplies dispensing and administrationWhat We're Looking For:Interest in helping people with diabetes improve their health and wellbeingOCP Part A licensed pharmacist in good standing with current OCP Injection Certification2-3 years of Ontario pharmacy practice and diabetes education experience preferredCertified Diabetes Educator (CDE) certification preferredMust have previous retail pharmacy experienceKroll proficiency and EMR experience an assetExcellent verbal and written communication skillsExcellent interpersonal, organizational and time management skillsAbility to travel locally to other LMC sitesSouth Asian language competency (Punjabi, Hindi) preferredCompensation range: The expected compensation for the position is $80K-$92K/annually.
