The primary function of the Long-Term Care Best Practice Implementation Coach is to support the implementation of best-practice guidelines in the Long-Term Care (LTC) sector across various regions of Ontario. RESPONSIBILITIES: Develop a regional plan to support the implementation of BPGs in the assigned Long-Term Care homes, outlining strategies to facilitate spread within the region and outreach to area long-term care homes. Seek, create and coordinate opportunities for promotion and integration of evidence-based best practices in the long-term care sector. Develop methodologies for promoting and supporting knowledge
