These services include health promotion, illness prevention, assessment, treatment/management, education, care navigation and advocacy. The mental health counsellor provides services to individuals, couples and families, and works with the family physician and other members of the health care team to provide comprehensive care.Roles and Responsibilities:Conducts a comprehensive assessment of individuals of all ages, couples and families referred by the family physician or other members of the health care teamProvides evidence-based treatment/management of mental health problemsImplements current best practice guidelinesMonitors client goals and outcomes through ongoing assessment and documentation of treatment progressResponds to client telephone calls and provides telephone advice, where appropriateConsults with other members of the health care team involved in the care of clientsMonitors and manages referrals from the family physician and other team members to ensure timely access to servicesCo-ordinates referrals and facilitates access to other health care providers and community programs and services, as appropriateLiaises and consults with community programs and services,
