Conduct comprehensive assessments of psychosocial needs




Create and monitor individual care plans ensuring person-centered care
Lead therapeutic and educational group sessions

Support residents and families in navigating care decisions Requirements:
Bachelor’s degree in Social Work (BSW) required
Registered with the Ontario College
Experience in long-term care settings
