Be a vital part of expanding Day Treatment services through home-based support and community connections. Responsibilities include establishing rapport with participants, facilitating educational groups, and assisting in accessing medical services.

This position requires travel across the Waterloo & Wellington Region and a valid G class driver’s license.

Key Responsibilities:
Maintain supportive relationships with clients
Facilitate psychoeducational and recreational activities
Support participants in accessing necessary healthcare




Engage in risk assessments and crisis management
Ensure accurate documentation and administrative work

Requirements:
Minimum education of College Diploma or University Degree in relevant fields
Familiarity with substance use dynamics and recovery strategies
Experience in social services is advantageous
Valid G class driver’s license essential
Must pass a Police Records Check for vulnerable sector work
