Transitional Case Manager - Addiction, Substance Use Disorder Service (SUDS) Direct Service - (OPSEU) Part-Time, Permanent *two positions available* London, Ontario (London Zone) This posting is the result of an existing vacancy - Application Deadline: October 6, 2026 - The Addiction Transitional Case Manager will perform duties and responsibilities consistent with providing a psychosocial model of care to assist in streamlining access to community addiction and mental health supports for individuals who are presenting to hospital services. This is a front-line, clinical position providing crisis intervention, brief addiction transitional case management, and system navigation related to substance use health. This position provides support and care pathways to community-based addiction and mental health services in a collaborative manner to reduce avoidable, unnecessary and repeat emergency department visits or admissions. Staff will support the ATCM program as required and will receive referrals and support the London Health Sciences Centre after receiving referrals to connect inpatient, outpatient or recently discharged clients to Substance Use Health services. The ATCM will liaise with LHSC staff as required which may include Nurse Practitioner’s, Physician’s, Nurses, Orderly’s, Clerks, Social Workers and Peer Harm Reduction Specialists from London Health Science Centre or other partnering agencies. The ATCM will work primarily in the Middlesex community and within London Health Sciences Centre supporting individuals to connect to CMHA TVAMHS and other community addiction programs. Successful candidates must have a Police Vulnerable Sector Check (PVSC) completed in the last six months. WHAT WE'RE OFFERING: What you'll be making - Starting rate for this role is$27.14 per hour (Level 1 – Direct Service), with a wage differential, based on regulation ($30.01 per hour for Level 1 – Direct Service, Regulated): Successful candidates who are registered with one of the designated skilled colleges (e.G.
