About the Role

Right at Home Victoria is seeking a growth-oriented Care Planner This role bridges the gap between community advocacy and client care. We believe in relationship-driven networking and high touch care management for families in our community. You will connect with local doctors, hospital discharge planners, and community groups to ensure families get the support they need.

As a qualified with a Healthcare or community support background (such as an HCA, LPN, or Medical Office Assistant looking for a career growth path) you bring transferrable knowledge and skills to work with families and the broader community representing Right at Home. We are looking for an outgoing curious person who is interested in building relationships and learning how best to support the unique needs of families and community partners and build meaningful relationships with both those we serve and others in their circle of care.

Core Responsibilities

Community Engagement

- Community Relations and Partnership Building
- Local Networking: Connect daily with healthcare professionals, medical clinics, and community groups to position us as their trusted partner.
- Referral Management: Build authentic, long-term relationships with local influencers to generate incoming client inquiries.
- Educational Presentations: Lead dynamic one-on-one meetings and group presentations to educate community sources on our specialized services.
- Brand Ambassadorship: Represent our organization at professional networks, wellness fairs, trade shows, and community events.




- Maintain consistent records of outreach activities and relationship progress using our internal platform.

Care Planning & Client Intake

- Inquiry Guidance: Respond to inbound inquiries from families, acting as a compassionate guide through their care options.
- Consultations: Conduct detailed client assessments and personal home consultations to establish trust and safety plans.
- Care Plan Design: Develop customized care plans and collaborate with internal scheduling teams to initiate care
- Recruitment hiring and ongoing support of caregivers
- Ongoing client care management through communication pathways with both caregivers and clients to assure care evolves as a client's needs change and to support therapeutic relationship building and an exceptional care experience

- An Exceptional Communicator: You are comfortable presenting to professional groups and speaking with medical professionals or anxious family members alike.
- Empathetic & Organized: You can easily pivot from a high-energy networking meeting to a sensitive, compassionate discovery call with a family in need.

Required Qualifications

- Experience: Minimum 2 years of experience in healthcare, home care, care coordination, client services, business development, or a related field.
- Required Competencies: Ability to balance client-centered care planning with relationship development, community engagement, and business growth initiatives.
- Valid Driver’s License, a clean driving record, and access to a reliable vehicle for daily territory travel.
- Background Check: Clear Vulnerable Sector Check (VSC) issued within the past 6 months.
- Legal Status: Legally entitled to work in Canada.
