Duties

- Provide comprehensive patient care tailored to individual needs across various settings including hospital, home care, and senior living communities
- Administer medications and treatments following established protocols while ensuring patient safety and comfort
- Perform clinical procedures such as phlebotomy, wound care, mobility assistance, and vital sign monitoring with precision and compassion
- Collaborate with interdisciplinary teams using electronic health record systems like Epic and Cerner to document patient progress accurately
- Assist with case management activities including discharge planning, patient education, and coordination of services for optimal recovery outcomes
- Support specialized care areas such as pediatrics, dementia care, pain management, and senior care to meet unique patient requirements
- Conduct patient assessments using medical terminology knowledge to develop effective care plans and communicate clearly with patients and families

Experience

- Prior hospital experience or acute care background demonstrating ability to handle complex medical cases efficiently
- Experience working with electronic health records systems such as Epic or Cerner is highly preferred
- Proven track record in patient care roles within nursing homes or assisted living facilities
- Knowledge of medical terminology,



medication administration protocols, and laboratory procedures including phlebotomy
- Familiarity with pediatric care or home health environments is a plus
- Strong understanding of medical office procedures and documentation standards
- Experience working with diverse populations including children, seniors with dementia or chronic conditions, and patients requiring pain management or case management services
