Job Description:

§ Perform in-home assessments of elderly clients and develop/update individualized care plans independently and where required with coordination with Regional Case Coordinator.

§ Deliver nursing services such as wound care, medication management, chronic disease support, catheter care, etc., as per CNO standards.

§ Monitor client conditions, track progress, and document care accurately using approved software.

§ Provide clinical supervision and delegation to PSWs or non-regulated staff as needed.

§ Communicate with families, physicians, and allied health professionals to ensure coordinated care.

§ Ensure infection prevention and control (IPAC) measures are followed at all times.





§ Respond to urgent clinical issues, changes in condition, or emergency situations.

§ Comply with all Ministry of Health and CNO regulatory standards and The Elder Care Company’s policies and procedures.

Qualifications:

Required:

- Active registration and in good standing with the College of Nurses of Ontario (CNO)
- CPR & First Aid Certification (current and valid)
- At least 1 year of nursing experience (preferably in home care, geriatrics, or community health)
- Knowledge of Ontario’s Home and Community Care Support Services guidelines
- Robust documentation, assessment, and clinical judgment skills
- Valid driver’s license and access to a reliable vehicle
- Clear Vulnerable Sector Police Check.
- Fully vaccinated for COVID-19 (as per current Ontario Health requirements)

Preferred:

- Experience working with clients with dementia, mobility challenges, or palliative care needs
- Knowledge of electronic charting systems.
- Ability to speak a second language (e.g., Punjabi, French, Spanish, Mandarin) is an asset.
