Summary of Duties

Provision of Social Work services to clients and families in Odette’s Patient & Family Support Program caring for patients with breast cancer at an advanced level of practice within an interprofessional model. This includes:

- completion of psychosocial assessments;
- leading or participating in Advance Care Planning and/or Goals of Care conversations;
- provision of counselling and emotional support;
- conducting group work; locating and arranging resources;
- consultation/collaboration; teaching; participating in research;
- administration and professional development.

Qualifications/Skills

- Masters Degree in Social Work required.
- Must be a current member in valuable standing with the OCSWSSW.
- Minimum 5 years’ previous experience working in a health care setting preferred.
- Demonstrated Psychosocial Oncology knowledge preferred.




- Psychosocial provision of care to those with breast cancer preferred.
- Proven excellent assessment, counselling and problem solving skills essential.
- Superior communication, negotiation, mediation and collaborative interprofessional team skills required.
- Well-established knowledge of relevant health care system processes and community resources required.
- Superior time management, organization, and prioritization skills in addressing multiple tasks and timelines in a fast-paced environment required.
- Proven commitment to person and family-centred care essential.
- Demonstrated strong commitment to academic practice required
- Student education required
