Compass is a non-profit organization based in Sudbury Ontario that provides mental health and developmental services for children, youth, and families. Most programs serve individuals up to age 18, with select services available to older youth and young adults. Supports range from short-term counselling to ongoing therapy, psychological assessment, and diagnostic services. Compass is also the lead agency responsible for providing live-in treatment services for youth who need temporary residential support during significant life transitions.

Reporting to the Client Services Manager, the Youth Wellness Facilitator serves as the first point of contact for youth, families and community partners, coordinating a centralized intake process, supporting access to Hub services, and working collaboratively with an interdisciplinary team to provide responsive, youth and family centered care.

RESPONSABILITIES:

- Oversee the intake process for youth and families, including the use of YWHO’s electronic client information system and appropriate screening tools, and the use of My Wellness Passport to plan, document and gather feedback on all activities.
- Create and maintain a developmentally appropriate, non-judgmental, non-stigmatizing, safe, and welcoming hub atmosphere.
- Represent YWHO to youth, families, and service providers, and support an orientation to the hub and an introduction to YWHO’s electronic client information system and measurement based care.
- Support the transition of youth/families between the various Youth Wellness Hubs service pathways (Skills and Well-being, Community & Social Support and Clinical Services)
- Collaborate with the Youth Wellness Team and other hub service providers to provide seamless access to support for youth.
- Support engagement with youth and their families to support ongoing access to services and gather feedback and input onto all hub programs and services.




- Provide services and support to youth and their families in a way that is consistent with the values of the YWHO Model.
- Participate in YWHO Provincial Community of Practice for Youth Wellness Facilitator role.
- Provide access to services both in person, remotely (off site) or virtually (online, phone) as needed. This position may also require evening work.

SKILLS AND KNOWLEDGE:

- Proficiency working in a computerized environment, including Microsoft Office applications (Word and Excel).
- Demonstrated ability to maintain strict confidentiality and handle sensitive information with professionalism and discretion.
- Demonstrated ability to assess priorities, analyse and solve problems
- Strong organisational skills and ability to meet deadlines
- Ability to develop a thorough understanding of the Agency's programs, services, organizational structure, and operations.
- Ability to work collaboratively with other experienced professionals including school boards, colleagues of other health and social service agencies, local politicians, etc.
- Demonstrated ability to work both independently and collaboratively within a multidisciplinary team.
- Excellent organizational and written communication skills and the ability to work effectively in a fast-paced environment are essential.
- Excellent interpersonal skills in connecting/building relationships with youth, family/caregivers and service partners.
- Knowledge of approaches that promote health equity, including anti-oppressive and anti-racist practice, 2SLGBTQIA+ inclusion, community engagement, cultural safety, trauma-informed care, harm reduction,



and human rights and accessibility frameworks.
- Strong written and verbal communication skills; bilingual (English/French) preferred

QUALIFICATIONS:

- Diploma or degree in social services or a combination of education and experience
- Minimum 1-year experience working with children and youth in a community based mental health/substance use setting.
- Experience in supporting safe and meaningful participation and being open to diverse ways of thinking about, understanding and articulating challenges related to mental health and substance use is required.
