CD Specialist – Flexible Assertive Community Treatment Team (Full Time Permanent)

The Role and Opportunity

CMHA Toronto is seeking a Concurrent Disorders Specialist to join our Flexible Assertive Community Treatment (FACT) Team. As an integral member of an interdisciplinary team, the Concurrent Disorders Specialist provides recovery-oriented assessment, intervention, consultation, and treatment services to adults living with serious and persistent mental illness and concurrent substance use concerns Working within a recovery-oriented, trauma- and violence-informed, culturally responsive, and person-centred framework, the Concurrent Disorders Specialist supports individuals in achieving their recovery goals through evidence-based addiction treatment, harm reduction, rehabilitation, and community integration strategies. In addition to direct service delivery, the Concurrent Disorders Specialist serves as a clinical resource to FACT team members, providing consultation and expertise related to concurrent disorders, substance-use treatment approaches, harm reduction strategies, and relapse prevention planning.

Location: 700 Lawrence Ave. W, and in the community

Hiring Salary Range: $58,783 - $64,407

Hours of Work: 35 per week

Key Responsibilities

- Conduct comprehensive assessments and develop individualized recovery-oriented service plans for adults living with serious mental illness and concurrent substance use concerns.
- Provide evidence-based concurrent disorders treatment, including motivational interviewing, harm reduction, relapse prevention, supportive counselling, and psychoeducation.
- Deliver community-based case management, rehabilitation, and recovery support services.
- Build therapeutic relationships that promote engagement, self-determination, wellness, and recovery.
- Support clients in accessing housing, healthcare,



income supports, community resources, education, and employment opportunities.
- Facilitate concurrent disorders groups and provide education to clients, families, caregivers, and natural supports.
- Provide crisis intervention, risk assessment, safety planning, and stabilization support as part of an interdisciplinary team.
- Serve as a concurrent disorders resource to FACT team members through consultation, clinical guidance, and knowledge sharing.
- Collaborate with hospitals, addictions services, healthcare providers, housing organizations, and community partners to support coordinated care.
- Participate in team meetings, quality improvement initiatives, and ongoing program development to support excellent client outcomes
- Ability to work independently, within the community working with population with complex needs

Experience & Education Required

CMHA Toronto’s ideal candidate will have:
- Bachelor's degree in Social Work, Psychology, or a related health or social services discipline.
- Registration with a professional regulatory body, where applicable, is considered an asset.
- Minimum three (3) years of experience supporting adults with serious mental illness and concurrent substance use concerns in a community-based setting.
- Experience providing addictions treatment, harm reduction services, and recovery-oriented care.
- Knowledge of concurrent disorders, trauma-and-violence-informed practice, motivational interviewing, relapse prevention, and recovery-oriented approaches.
- Strong assessment, counselling, engagement, and service planning skills.
- Demonstrated experience providing crisis intervention, risk assessment, and community-based support.
- Strong interpersonal, communication, and relationship-building skills, with the ability to work effectively within an interdisciplinary team.
- Knowledge of community mental health, addictions, healthcare, housing, and social service systems.
- Valid Ontario Driver's Licence and access to a reliable vehicle for community-based service delivery.
