Nurse Case Manager 2 (Registered Nurse) – Flexible Assertive Community Treatment Team ( Full Time Permanent )

The Role and Opportunity

CMHA Toronto is seeking a Registered Nurse to join our Flexible Assertive Community Treatment (FACT) Team. As a member of an interdisciplinary team, the Registered Nurse provides recovery-oriented, trauma- and violence-informed nursing assessment, treatment, education, and care coordination to adults living with reliable and persistent mental illness and concurrent disorders. Services are delivered primarily in community settings and focus on promoting recovery, psychiatric stability, physical health, community integration, and quality of life.

Working collaboratively with clients, families, healthcare providers, psychiatrists, and community partners, the Registered Nurse plays a key role in supporting client-centred care and recovery while contributing to coordinated and responsive service delivery within the FACT model.

Location: 700 Lawrence Ave. W, and in the community

Hiring Salary Range : $68,975 - $75,581

Hours of Work: 35 per week

Key Responsibilities

- Conduct comprehensive assessments and develop individualized recovery-oriented service plans for adults living with serious mental illness and concurrent substance use concerns.
- Provide evidence-based concurrent disorders treatment, including motivational interviewing, harm reduction, relapse prevention, supportive counselling, and psychoeducation.
- Deliver community-based case management, rehabilitation, and recovery support services.
- Build therapeutic relationships that promote engagement, self-determination, wellness, and recovery.
- Support clients in accessing housing, healthcare, income supports, community resources, education, and employment opportunities.




- Facilitate concurrent disorders groups and provide education to clients, families, caregivers, and natural supports.
- Provide crisis intervention, risk assessment, safety planning, and stabilization support as part of an interdisciplinary team.
- Serve as a concurrent disorders resource to FACT team members through consultation, clinical guidance, and knowledge sharing.
- Collaborate with hospitals, addictions services, healthcare providers, housing organizations, and community partners to support coordinated care.
- Participate in team meetings, quality improvement initiatives, and ongoing program development to support excellent client outcomes
- Ability to work independently, within the community working with population with complex needs

Experience &

Education Required

CMHA Toronto’s ideal candidate will have:

- Bachelor of Science in Nursing (BScN) and registration in good standing with the College of Nurses of Ontario.
- Minimum two (2) years of experience working in mental health, preferably within a community-based setting.
- Experience assessing and supporting adults with mental health issues and disorders, including conducting bio-psycho-social assessments and mental status examinations.
- Experience providing counselling and education to clients and their families regarding medications, including their use and effects.
- Advanced knowledge of psychotropic medications, including their effects and the administration of medications as required within the scope of nursing practice.
- Knowledge and experience administering and scoring mental health assessment tools appropriate to the nursing profession.
- Knowledge of and demonstrated commitment to recovery-oriented approaches in mental health care.
- Experience supporting individuals with addictions, forensic involvement, and/or dual diagnosis is considered an asset.
