Occupational Therapist, MSc – Flexible Assertive Community Treatment Team (Full Time Permanent)

The Role and Opportunity

CMHA Toronto is seeking an Occupational Therapist to join our Flexible Assertive Community Treatment (FACT) Team. As a member of an interdisciplinary team, the Occupational Therapist provides recovery-oriented occupational therapy assessment, intervention, consultation, and rehabilitation services to adults living with serious and persistent mental illness and concurrent disorders. Working within a recovery-oriented, trauma-and-violence-informed, culturally responsive, and person-centred framework, the Occupational Therapist supports individuals in achieving their recovery goals through functional assessment, skill development, community integration, vocational and educational support, and interventions that promote independence and meaningful participation in daily life.

The Occupational

Therapist also collaborates with FACT team members and community partners to develop and implement individualized service plans that enhance functioning, well-being, and quality of life.

Location: 700 Lawrence Ave. W, and in the community

Hiring Salary Range: $63,879 - $69,994

Hours of Work: 35 per week

Key Responsibilities

- Conduct occupational therapy, functional, and intake assessments to identify client strengths, needs, and recovery goals.
- Develop and implement individualized, recovery-oriented service plans in collaboration with clients and the interdisciplinary team.
- Deliver evidence-based occupational therapy interventions that promote independence, daily functioning, and community participation.
- Assess and teach independent living skills, including self-care, household management, budgeting,



transportation, and nutrition.
- Support clients in managing mental health symptoms, medication routines, and overall health and wellness.
- Provide vocational, educational, and employment supports, including work readiness and workplace accommodation planning.
- Facilitate social skills development, community integration, recreational engagement, and supportive counselling.
- Connect clients and families with community resources, natural supports, healthcare services, and housing supports.
- Participate in crisis prevention, intervention, discharge planning, and ongoing care coordination.
- Maintain accurate clinical documentation and contribute to quality improvement, client safety, and interdisciplinary team initiatives.

Experience & Education Required

CMHA Toronto’s ideal candidate will have:
- Master's degree in Occupational Therapy.
- Registration in positive standing with the College of Occupational Therapists of Ontario.
- Minimum three (3) years of experience providing occupational therapy services to adults with serious and persistent mental illness, including individuals with concurrent mental health and substance use concerns.
- Experience working in community mental health, psychosocial rehabilitation, and recovery-oriented practice.
- Knowledge of evidence-based occupational therapy interventions, functional assessment, and independent living skills training.
- Experience supporting community integration, vocational and educational goals, and rehabilitation planning.
- Ability to work collaboratively within an interdisciplinary team and with community partners.
- Strong assessment, documentation, crisis intervention, and care coordination skills.
- Valid Ontario driver's licence and access to a reliable vehicle for community-based service delivery.
