Supportive Housing Worker – Adaptable Assertive Community Treatment Team (Full Time Permanent)

The Role and Opportunity

CMHA Toronto is seeking a Supportive Housing Worker (Housing Support Worker) to join our Flexible Assertive Community Treatment (FACT) Team. The Supportive Housing Worker plays a key role in supporting clients to secure, maintain, and thrive in stable housing while promoting recovery, independence, and community integration. Through housing-focused case management, life skills coaching, advocacy, and collaboration with community partners, the Supportive Housing Worker assists individuals living with serious mental illness and concurrent disorders to build the skills, supports, and resources needed to live successfully in the community.

Location: 700 Lawrence Ave. W, and in the community

Hiring Salary Range: $54,943 - $60,203

Hours of Work: 35 per week

Key Responsibilities

- Support clients in obtaining, maintaining, and transitioning within housing through assessments, tenancy support, and housing stabilization services.
- Assist clients in developing independent living skills, including budgeting, meal preparation, medication management, housekeeping, and personal care.
- Provide recovery-oriented coaching, mentoring, and practical supports that promote independence and quality of life.
- Coordinate move-ins, lease signings, housing inspections, rent reviews, and move-out processes in collaboration with clients and housing partners.
- Develop and maintain positive working relationships with landlords, property management staff, community agencies, and service providers.
- Facilitate housing meetings, group activities,



and community engagement opportunities that support social inclusion and well-being.
- Connect clients with healthcare, psychiatric, social, and community resources and support attendance at appointments as needed.
- Provide crisis intervention, conflict resolution, harm reduction support, and tenancy preservation services.
- Conduct unit inspections, monitor health and safety concerns, and coordinate maintenance, repairs, and property-related issues.
- Maintain accurate documentation and actively participate in team meetings, service planning, quality improvement, and client safety initiatives.

Experience & Education Required

CMHA Toronto’s ideal candidate will have:
- Degree or diploma in Social Work, Social Service Work, or a related field.
- One (1) to four (4) years of experience supporting individuals living with mental illness, concurrent disorders, homelessness, and/or justice system involvement.
- Knowledge of recovery-oriented practice, trauma- and violence-informed care, harm reduction, psychosocial rehabilitation, and supportive housing principles.
- Familiarity with community resources, referral processes, and client engagement approaches such as motivational interviewing.
- Knowledge of the Residential Tenancies Act and principles of housing stability and tenant support.
- Strong assessment, crisis intervention, problem-solving, communication, and relationship-building skills.
- Ability to work effectively with diverse communities using an anti-oppressive, anti-racist, and person-centred approach.
- Ability to work independently, manage competing priorities, and collaborate within an interdisciplinary team.
- Proficiency in electronic documentation systems and standard workplace technology.
- Valid Ontario driver's licence and ability to travel within the community as required.
