As a Transition Facilitator, you will support Indigenous communities by addressing challenges around transitioning care from hospitals back to their home environments. This role collaborates closely with health service providers to implement a community-focused approach to health care, integrating both traditional and up-to-date practices. Key Responsibilities:
• Ensure compliance with Ontario Quality Standards
• Collaborate with stakeholders for continuous care




• Facilitate client and family engagement in care planning
• Build capacity through training and mentoring
• Document and report on program impacts
Requirements:
• Relevant post-secondary degree in health/social services
• 3-5 years of case management in community settings
• Excellent organizational and communication skills
• Experience with Indigenous culture and practices
• Valid driver’s license and ability to travel
