Health and Wellness Manager, LPN Relocation Required.

Location: Surrey, BC

Salary: $80,000–$90,000 per year + performance-based bonus

About the Role

We are seeking an experienced Health and Wellness Manager, LPN to lead resident care, wellness programs, regulatory compliance and staff development within a senior living community in Surrey.

The successful candidate will be responsible for ensuring high-quality, compassionate, and resident-centered care while maintaining compliance with BC Assisted Living requirements.

The successful candidate will be comfortable working in a culturally diverse community and supporting residents and families who may communicate primarily in a language other than English.

Key Responsibilities

- Develop and maintain individualized Personal Service Plans and resident health records.
- Conduct resident assessments and reassessments and coordinate hospital discharges.
- Manage incident reporting and ensure regulatory compliance.
- Liaise with the Health Authority regarding inspections, licensing, and compliance.
- Lead family care conferences and communicate with residents and families regarding care plans.
- Train, mentor, and support care staff.
- Develop and coordinate wellness programs and resident activities.
- Monitor care and wellness KPIs and identify opportunities for improvement.





What You Bring

- Current LPN License in British Columbia
- Diploma/certificate in Health Care Administration, Gerontology or Community Support Work.
- Minimum 3 years of experience in assisted living, long-term care or a similar seniors' care environment.
- Strong knowledge of PSPs, resident assessments, incident reporting, and BC regulatory requirements.
- Familiarity with Health Authority processes.
- Experience with EHR systems and Microsoft Office.
- Robust leadership, communication, organizational, and interpersonal skills.
- Compassionate, culturally sensitive, and resident-focused approach.
- Ability to occasionally work evenings or weekends as required.

Pay: $80,000.00-$90,000.00 per year
