The role utilizes a population health approach via counseling, group work, education, health promotion, advocacy, chronic disease management, and resource linkage, focused on client and family‑centered care.Key ResponsibilitiesAssess, plan, and provide supportive counseling to individuals and groups.Maintain clinical records and documentation.Participate in interdisciplinary care planning.Prepare and present information and education sessions.Provide mentoring and support for students.Participate in developing, maintaining,
