This posting is for a current vacancy.Location:HNHB boundariesProgram:Integrated Comprehensive Care Program (ICCP) - Division:Clinical Programs - Regular/Temporary:Regular - Full/Part Time/Casual:Part TimePosting Date:May 29, 2026 - Closing Date:Until Filled - Wages:Non-Union Wage Grid $22.98-$50.62 - French Language Skill Requirement:N/APosition SummaryThe Dietitian (D-CP) provides the highest quality of nutritional care to optimize the nutritional status of program clients in accordance with individual care plans, SJHC standards, the College of Dietitians of Canada and the Ministry of Health guidelines.ResponsibilitiesSupport clients with a wide variety of special dietary needs, various health conditions, mental health and/or addictions and/or mobility issues.Approach work guided by the Client Bill of Rights, nutritional best practices, culturally sensitive approaches, and safety.Provide direct support to clients enrolled in streams within the ICCP and St Mary’s health programs.Support managers and frontline staff with dietary needs and services; collaborate with clients, families, caregivers, care teams and other interdisciplinary functions.Work with clinical program clients who are discharged from hospital but require ongoing care; engage with a multidisciplinary health care team.Participate in client reviews, palliative care, and case conferences.Educate clients and staff on therapeutic and general nutrition; document issues as prescribed.Develop and implement individualized service plans in collaboration with others.Advocate for clients and assist with connecting to community-based services as related to food security needs.Build relationships with community agencies and cultural organizations; participate in community initiatives.Perform preliminary swallowing assessments and liaise with the community swallowing assessment team as required.Remain compliant with relevant legislation; maintain privacy and confidentiality of all health records.Other duties as required.Core CompetenciesDemonstrates mission, vision and values; delivers compassionate care, respects diversity.Works collaboratively with an interdisciplinary team and promotes a healthy, inclusive work environment.Effective problem solving, analytical thinking and creative solutions to complex problems.Strong communication skills, both verbal and written; customer service oriented.Positive, professional attitude; willingness to learn, mentor, and go the extra mile.Ability to prioritize, meet deadlines in a rapid-paced environment; adhere to policies, procedures and safety standards.Maintain privacy and confidentiality with electronic medical records.QualificationsUniversity-level degree in a related field (B.Sc., Home Economics, Food and Nutrition, etc.) - requiredCompletion of a dietitian internship - requiredUpdated and unrestricted registration with a professional college (College of Dietitians) - requiredExperience with geriatric populations - preferredExperience with mental health/addictions population – assetMinimum 3 years’ experience in a similar role - preferredExperience with vulnerable clients in community care settings - preferredGentle Persuasive Approach training – assetProficient computer skills (Excel, Office 365, Alayacare, etc.) - preferredSt. Joseph’s Home Care is an equal opportunity employer committed to equity, inclusiveness, and diversity across all programs, practices, facilities, and team.
