Key Responsibilities
Identify and analyze seniors' individual service needs
Create supportive community relationships for seniors
Collaborate with social service providers for comprehensive support

Requirements:
Diploma or Degree in Human Services, Social Work, or similar
2–4 years of relevant experience preferred
Solid communication skills necessary
Vehicle and valid driver's license mandatory
