Become part of the NECHC Mobile Outreach Street Health team as a Community Outreach Nurse, focusing on primary care for the homeless and those in need. Engage in health promotion and harm reduction in the community. In this advanced nursing position, you will report to the MOSH Manager and collaborate closely with the outreach team.

Your role involves assessing and addressing the health needs of street-involved individuals through direct care and program-level consultation. A commitment to wellness and prevention, guided by the social determinants of health, will be central to your approach. Key Responsibilities:
Provide outreach nursing services to homeless individuals

Focus on relationship-based healthcare delivery Requirements:
Active RN license in the region
Experience in community or outreach health
Knowledge of harm reduction frameworks
