Conducting comprehensive health assessments and medical historiesDiagnosing, treating, and managing acute and chronic conditionsPrescribing medications and managing polypharmacyParticipating in care conferences and communicating with familiesProviding clinical leadership and support to the nursing and PSW teamsEducating staff on disease prevention, infection control,
and best practicesSupporting hospital diversion initiatives and continuity of careCollaborating with physicians, specialists, allied health, and administrationParticipating in emergency response and on‑call rotations (as agreed upon)Requirements
We’re looking for someone who brings both clinical excellence and a deep commitment to compassionate senior care. The ideal candidate will have:A valid Nurse Practitioner license in Ontario and active CNO registrationAn OHIP billing number and comfort operating as an independent contractor3+ years of NP experience (preferably in long‑term care or geriatrics)A background in complex care, dementia, and chronic disease managementA team-oriented, educational mindset to guide team learning and developmentStrong clinical judgement, communication, and family engagement skillsFlexibility to work full‑time or part‑time and openness to on‑call support
