About Aiyana Services — Aiyana Services supports children, adults and seniors in their own homes. We provide in-home personal care, respite and community supports, funded through Family Support for Children with Disabilities (FSCD), Persons with Developmental Disabilities (PDD), Alberta Health Services home care hours billed through Alberta Blue Cross, and private pay. We deliver care in English, Somali and Kiswahili.

We are based in Edmonton and are now growing into Calgary. — The role — This is the clinical seat in a growing agency, and it is a role you will shape rather than inherit. You will set the standard for the care our support workers deliver in the home, keep our clients' care plans current in both cities, and be the person families, case managers and community organizations meet first. — What you will do: Assess and plan — complete client assessments and build individualized care plans with the family. Oversee the care — provide clinical oversight and coaching to support workers in the home.

Review care plans remotely — complete client and care plan reviews by phone and video, including for our Calgary clients, and update the plan when needs change. Coordinate the team — keep support workers, families and the office working from the same care plan, so every change reaches the worker in the home before the next visit. Hold the standard — review documentation and progress notes against what our funders require.

Work the funder side — deal directly with FSCD,



PDD and Alberta Health Services case managers on client needs and changes.

Represent

Aiyana — build relationships with families, doctors' offices, schools and community organizations. Build the team — support recent staff training and competency sign-off alongside our onboarding program. — What you bring: an active practice permit with the College of Licensed Practical Nurses and Health Care Aides of Alberta. Home care or community care experience. A valid driver's licence and a reliable vehicle. Current CPR level C and a criminal record check with vulnerable sector search. Comfort working independently in a small team where your judgement carries weight, and confidence running client reviews by phone and video. — Nice to have: a second language.

Experience with FSCD, PDD or client-directed home care.
