**Position Summary**
As a member of the

Family Navigation Hub (https://hollandbloorview.ca/services/programs-services/family-navigation-hub) team, the Program Coordinator supports the operational coordination and administration stewardship of Holland Bloorview's food security program and related initiatives.

In partnership with clients, families, staff, and community partners, the Program Coordinator ensures effective implementation of program activities and related workflows, maintains operational processes and standards, and contributes to reporting, evaluation, and continuous improvement.

This is a rapid paced, dynamic work environment requiring the ability to work independently and collaboratively with inpatient and outpatient teams to strengthen access to food supports for clients and families and advance care that's socially accountable.
**Key Responsibilities**
**Program Coordination**
Oversees and supports day-to-day administration and implementation of the food security program and related initiatives.





Ensures compliance with operational processes and workflows, actively contributing to their ongoing development and refinement.

Identifies operational constraints and proactively brings forward recommendations to leadership.

Builds relationships with internal partners to foster collaboration and ensure high-quality program delivery.

Optimizes inventory management and coordinates the efficient distribution of food supports to clients and families.

Serves as a primary point of contact for families, providing clear communication to ensure access to food supports.

Coordinates food program expenditures, processes financial reconciliations, and maintains documentation to ensure accurate budget tracking.

Fosters innovation and creativity across program delivery in response to client and family needs.

Contributes actively to team and organizational meetings by sharing program updates.

Performs other duties as assigned or required.
**Data, Evaluation & Reporting**
Compiles program impact data, including statistics and qualitative feedback, to assist in preparing reports and presentations to diverse internal and external audiences.

Measures and evaluates program impact by supporting data collection and reporting under the support and direction of the Senior Director.
**Communication, Promotion & Outreach**
Coordinates the production and distribution of program flyers, marketing materials, and other promotional resources.

Develops and updates key communication deliverables, including project updates, educational resources, website content, and multimedia assets (e.g. photographs) for annual reporting.

Co-designs community food resources in partnership with the Client and Family Integrated Care team.
**Minimum Knowledge, Skills, and Abilities**
Undergraduate degree in Community Health, Public Health, Social Work, or a related social sciences/health discipline; an equivalent combination of education and relevant experience will be considered.

Demonstrated track record of co-designing and delivering equity-driven initiatives in partnership with diverse communities to address social determinants of health.

Minimum of 2 years of experience working with children, youth, and families within a paediatric or community health setting preferred.

Strong understanding and demonstrated ability to apply health equity,



anti-oppression, and population health frameworks, including how social determinants of health impact client and family experience.

Proven ability to collect, analyze, and apply data to evaluate programs and initiatives, inform decision-making, and drive continuous improvement.

Proficient with design and communication tools (e.g., Canva, Microsoft Publisher) to develop accessible communication materials and digital assets.

Demonstrated capability to effectively prioritize tasks, manage competing deadlines, and deliver high-quality work in a dynamic setting.

Collaborates effectively within an interdisciplinary team to deliver client-and-family-centred care.

Highly self-directed with demonstrated initiative and the ability to work independently with minimal supervision.

Demonstrated commitment to professional development and continuous learning.
