**The Position**
The Child and Youth Worker on the Rehabilitation and CCC team is responsible for supporting clients with their behavioural needs, creating and implementing existing behavioural plans to enhance their participation in therapy and ability to cope with their inpatient admission.

The Child and Youth Worker provides support to clients, caregivers, and members of the interdisciplinary team.

Responsibilities may include providing individualized 1:1 supervision and behavioural support to clients across all inpatient programs.
**Shift Schedule:** Please note that this is a permanent evening position with working hours from 1500 to 2300 hours (3:00 PM – 11:00 PM).
**Clinical Practice:**
Consults and collaborates with members of the interdisciplinary team, families, and community providers to support client goals and needs.

In collaboration with the Behaviour Analysts and other members of the interdisciplinary team, assesses the behavioural needs of clients and develops, implements,



and monitors behaviour plans that are goal-directed, evidence-based, and outcome-focused.

Provides individualized 1:1 behavioural support to clients and identifies, advocates for, and supports the need for ongoing 1:1 service through additional funding when required.

Responds to and assist with de-escalating clients experiencing behavioural crises while adhering to the organization's Code White procedures and safety protocols.

Supports and respects clients' and families' rights to make informed decisions based on their individual values, beliefs, preferences, and experiences.

Provides care and support in accordance with established organizational policies, procedures, standards, and best practices.

Completes accurate and timely documentation, including behavioural assessments, progress notes and discharge summaries, in accordance with Holland Bloorview's documentation policies.

Advocates for the best interests, rights, safety, and well-being of clients and families as required.

Participates in client family team meetings, interdisciplinary meetings, and multidisciplinary rounds.

Provides supervision, guidance, and mentorship to Child and Youth Worker students.

Records and maintains accurate workload statistics and other required service data.

Attends and actively participates in staff meetings and team discussions.

Participates in quality improvement initiatives and contributes to the organization's academic agenda through education, knowledge translation, and/or research activities.

Supports and promotes a safe, healthy, and inclusive work environment and contributes to a culture of client and patient safety through daily practice.

Provides education and coaching to clients, families, staff, students, and, where appropriate, community professionals regarding behavioural strategies, interventions, and behaviour support plans.

Communicates effectively and in a timely manner with clients, families, team members, and community partners.

Maintains current knowledge of professional literature, evidence-based practices, and emerging approaches relevant to behavioural support and child and youth care.

Performs other related duties and responsibilities as assigned.
**Research:**
Participates in research activities and initiatives, as applicable to the role and clinical setting.

Applies relevant research findings and current evidence to inform and support evidence-based practice and client care.
**Education:**




Child and Youth Worker Degree preferred (Diploma with relevant experience will be considered)
Maintains current certificate in Basic Cardiopulmonary Resuscitation Life Saving course (BCLS)
Maintains current Crisis Prevention and Intervention certification
**Licence/Memberships:**
A member of The Ontario Association of Child and Youth Care (OACYC) is required
Child and Youth Worker Skilled Certification preferred
**Experience:**
Minimum 2 years' previous job-related experience working with clients and families with a variety of disabilities
Experience working in an interdisciplinary hospital or health related setting is preferred.
**Skills:**
Works as a member of the interdisciplinary team
Ability to handle crisis situations calmly
Demonstrates ability to organize time and meet deadlines
Demonstrates initiative and self-direction
Demonstrates ability to effectively and efficiently manage fluctuating clinical responsibilities.
