- Identify drug therapy problems with patient collaboration
- Provide documentation and reporting on medication management




- Educate patients and teams on drug-related issues
- Support process improvements in patient safety
- Conduct mentoring sessions for pharmacy students and residents Requirements:
- Registration with College of Pharmacists of BC
- Two years of patient care experience required
- Valid Class 5 driver’s license necessary
- Proven communication and organizational skills
