Be a vital part of expanding Day Treatment services through home-based support and community connections. Responsibilities include establishing rapport with participants, facilitating educational groups, and assisting in accessing medical services.

This position requires travel across the Waterloo & Wellington Region and a valid G class driver’s license.

Key Responsibilities:

- Maintain supportive relationships with clients
- Facilitate psychoeducational and recreational activities
- Support participants in accessing necessary healthcare




- Engage in risk assessments and crisis management
- Ensure accurate documentation and administrative work

Requirements:

- Minimum education of College Diploma or University Degree in relevant fields
- Familiarity with substance use dynamics and recovery strategies
- Experience in social services is advantageous
- Valid G class driver’s license essential
- Must pass a Police Records Check for vulnerable sector work
